Great Homer Street Medical Centre take great care and pride for all our patients when registering, are given an excellent service and provide the right care for everyone therefore please note we will need to see the following documents:
- 1. Completed and signed the purple form (GMS1)
- 2. Passport,valid Photo I.D. or Driving Licence
- 3.Utility Bill and / or Rent Agreement / Mortgage contract with the correct name and address as proof of residency if possible
- 4. Please call in for a full New Patient Registration Pack or download from the link below, print, sign and call into reception will all the documents. The Receptionist / Admin staff member will arrange an appointment with the Practice Nurse to help get your details on the system including medication if necessary.
A New Patient Check with the Practice Nurse usually takes place before seeing the doctor. This is a 30 minute appointment and allows the nurse to update any medical treatment or conditions you may have. It will also allow the practice to invite you for an annual check up if necessary.
Patient Notice - Patient Records Access
From their 13th Birthday patients will be allowed to manage their own health records. This means parent's using the online service on a child's behalf will have their access revoked, until the patient gives their consent. Children aged 13 and over can either nominate parents as a proxy user or manage the account themselves.
We apologise for any inconvenience this may cause. This new policy will bring us in line with new legislation, and is intended to work in the best interest of all our patients. If you have any questions or concerns, please do not hesitate to contact a member of our reception team.
